  1. Once logged into the Datatrak Direct application, the Patient Portal dashboard will be displayed. 

  2. Forms can either be seen immediately on the screen or accessed via the "Show My Forms" button at the top of the screen.

  3. If consent is required, the following message will display if you attempt to access your forms

    1. To provide consent, tap the eConsent option in the menu. Once completed, depending on the study, you may receive an email verification with attached documents.

  4. From there you will receive notifications as forms become due to complete. This varies by study.

  5. If available, some studies may allow for revoking consent from the app. This can be done via the Icon at the top of the consent form:

    1. To revoke consent, a reason will be requested. Simply enter a reason and tap "Revoke Consent". This will disable any further data entry until consent is provided again.



My Completed Forms (App only)

Depending on the study configuration the My Completed Forms option will appear under the Patient’s Profile. The completed forms section will display the ePRO forms that have been saved to the patient casebook. When in the My Completed Forms manager the form can be opened and reviewed by selecting the desired form from the row of available forms. The date or date and time for the saved Visit Date.
