Summary

This article covers the host-level form builder.

It's important to understand, there are two form builders::

  • Host-level - Found under the Website Host menu
  • Study-level - Found under the Study menu

This article focuses on the host-level form builder.  

Forms developed at the Website Host Level are used for these applications only at the host level:


In each of the locations listed previously, you will find an example of a Profile form. However, you can add and customize forms as needed.


There are three types of forms to correspond the applications named above:

  • User
  • Site
  • Study

When creating a new form, be sure to select the appropriate type.
